Option 1-File By Yourself Online –

From a computer: Click Aflac SmartClaim or scroll down to log in and follow the instructions.

From a mobile device: Click SmartClaim Mobile App to download, log in and follow the instructions.

Option 2-File with Help from Omnia Benefits Agent – Send the following by email or fax to claims@omniabenefits.com or 443-740-9214:

1. Completed and signed claim form from links below
2. Completed and signed Authorization to Obtain Information Form giving us permission as Aflac agents to retrieve necessary medical documents on your behalf
3. All supporting documents

The biggest thing to remember is to collect any documentation possible from your doctor, hospital, emergency room, physician, chiropractor, dentist, or any other medical professional that shows what you were treated for, when, and why.

For example, if you are admitted into the hospital for any reason at all, you need the document that shows your admission time and date, as well as discharge time and date, along with the reason why you were admitted.  A UB04 document includes all information required to get your claim expedited the fastest.
